We provide a complete continuum of residential programs designed to support individuals with serious and persistent mental illnesses. These programs provide support and the proper amount of care to help residents live happy and successful lives. Residents can progress through the levels of residential care, as conditions dictate, always with the focus of the greatest independence possible.
We maintain over 150 apartments, in many of our service areas where individuals can live with independence but still have access to appropriate support and assistance.
A program for transitioning from inpatient to the community. Located in Yakima, this 12- resident home is staffed 24 hours a day to provide support, case management and to teach skills for independent living.
This facility is appropriate for longer-term stays for individual who need considerable assistance with performing daily activities. Gleed Orchard Manor provides a safe environment for those who require a longer-term residence, yet need only moderate levels of support. The 30-bed facility offers residents a comfortable, home-like environment.
A step down program from state hospital care. This 16-bed facility provides residential and treatment services for individuals requiring a high degree of support for their mental health conditions, yet not to a level that would require skilled nursing.
Garden Village is a skilled nursing facility (operated in partnership with Yakima Valley Memorial Hospital). Here individuals with complex medical and mental health needs receive local services as an alternative to large state run psychiatric facilities.
Pathways is a 16-bed residential treatment facility for adults with co-occurring mental health and substance use disorders. At this facility, residents receive individualized and integrated co-occurring disorder treatment from an interdisciplinary team of mental health and substance use disorder treatment professionals. The initial phase of treatment focuses on psychiatric stabilization, including medication management services when needed. Staffed 24 hours a day, Pathways provides intensive treatment to individuals who need a higher level of care than can be provided in outpatient co-occurring disorder treatment.
